Dental Insurance Plan
The company offers two dental insurance plans there is a PPO plan through Delta Dental and an HMO plan through Delta Care USA.

The HMO plan will require you to use a primary care dentist and all services must originate from this dentist. To change your primary care dentist please call 800-422-4234. Changes received by the 21st of the month will become effective the first day of the following month. |
The PPO plan will allow you to use any dentist, however, benefits are reduced for non-Delta Dental PPO dentist or “Out-of-Network” dentists. |

HOW TO FIND A DENTIST
To find a dentists visit www.deltadentalins.com and use the Find a Dentists search box. For Network select DeltaCare USA if you are interested in the HMO plan and Delta Dental PPO if you are interested in the PPO plan.
Please note it is advised that you call the dentist and confirm they are accepting new patients before you register the dentist as your primary care physician.
