The vision insurance plan is provided through Vision Service Plan (VSP). You can choose any optometrist, however, if you choose an out of network doctor benefits may be reduced and you may need to submit a claim for reimbursement.

To Find an Optometrist
To find an optometrist visit www.vsp.com and select Find A Doctor from the menu bar.
No ID Card Necessary
VSP does not issue ID cards, if you enroll in this coverage notify your optometrist that you have coverage through VSP. Your optometrist will be able to verify your coverage when you provide your personal information such as name, date of birth, and social security number.
